Chodzi o to, żeby mieć realną, łatwo dostępną wiedzę na temat zarobków lekarzy – zarówno tych, którzy pracują na kontrakcie w kilku różnych szpitalach, jak i tych, którzy są na etacie.

Czy to nie jest działanie czysto wizerunkowe Ministerstwa Zdrowia? Takie dane resort może chyba uzyskać w inny sposób, np. z Krajowej Administracji Skarbowej?
Takiej instytucji jak AOTMiT łatwiej te dane uzyskać, mając do tego delegację ustawową, niż ministerstwu z innych źródeł. Nie chcemy „śledzić” konkretnego człowieka, ale pokazać rzetelną analizę zmian zachodzących w systemie wynagradzania w ochronie zdrowia. Obecnie widzimy zarobki tylko na poziomie jednego podmiotu. Dla przykładu, według danych za ubiegły rok ok. 1 proc. lekarzy na kontrakcie zarabiało powyżej 100 tys. zł miesięcznie. Kłopot w tym, że dane te dotyczyły pojedynczych umów zawieranych przez lekarzy z konkretnym szpitalem. Po zsumowaniu przychodów z różnych placówek może się okazać, że takich osób z zarobkami powyżej 100 tys. zł miesięcznie jest w systemie od 5 do 10 proc.

Agencja jest przede wszystkim instytucją doradczą dla ministra zdrowia i to jemu musimy pokazywać te dane, bez dotychczasowych „gwiazdek” i klauzul, że są niepełne. Dzięki poprawce będziemy mogli łączyć w naszej bazie dane lekarzy po numerze PESEL lub numerze prawa wykonywania zawodu. Jestem zdania, że nie należy cały czas domagać się systemowej gwarancji wzrostu przychodów szpitali, tak, żeby mogły pokryć wszystkie koszty wynagrodzeń. Może te koszty wynagrodzeń wynikają czasami trochę z wynaturzeń? Dlaczego mielibyśmy gwarantować systemowo ich pokrycie na okoliczność tego, że np. pochłaniają 90 proc. przychodów szpitala i trzeba zagwarantować lekarzom takie nienaturalne przyrosty płac?
Wycena powinna odzwierciedlać koszty, ale może być też budowana metodą podażowo-popytową. Chodzi o to, by płacić więcej za to, co jest priorytetem zdrowotnym lub by zachęcić lekarzy do pracy w sektorze publicznym
To nie jest wrzutka legislacyjna Ministerstwa Zdrowia na czas walki z Naczelną Izbą Lekarską przed wyborami w samorządzie lekarskim?
Nie traktuję tak tego. Tak naprawdę delegacja do uzyskiwania danych o zarobkach po numerze PESEL była elementem szerszego pakietu zmian w ustawie. Nie udało się wprowadzić zmian w mechanizmie waloryzacji minimalnych wynagrodzeń personelu medycznego na etacie, ani przesunąć ustawowych podwyżek z 1 lipca na 1 stycznia, natomiast ten element również był w pakiecie zmian i po prostu jest dalej procedowany. W ramach Zespołu Trójstronnego ds. ochrony zdrowia mocno wybrzmiał głos strony pracodawców, że jest zgoda, żeby takie dane zbierać i je monitorować.

Co wynika z już zgromadzonych danych na temat zarobków personelu medycznego?
Z zebranych danych z lutego, które obejmują 96 proc. szpitali sieciowych, wynika, że średnie wynagrodzenie lekarza specjalisty na umowie o pracę wynosi 24 tys. zł brutto. Kwota ta uwzględnia wszystkie dyżury i dodatki stażowe. Co ciekawe, 75 proc. lekarzy na etatach zarabia do 29 tys. zł, a pozostałe 25 proc. powyżej tej kwoty.
Dlaczego wynagrodzenia kontraktowców rosną, skoro podwyżki dotyczą wynagrodzeń minimalnych dla personelu na etatach?
Obecnie nie realizuje się podwyżek „na PESEL”, czyli nie dajemy szpitalowi dokładnie tyle, ile dany pracownik powinien dostać według tabeli wynagrodzeń po waloryzacji. Zamiast tego, szacujemy zapotrzebowanie szpitali na wzrost wynagrodzeń i wrzucamy te środki w uśrednioną wycenę świadczeń. Dorzucamy po kawałku do różnych procedur, żeby wynik w szpitalach się zgadzał. Problem polega na tym, że każdy szpital ma inną strukturę świadczeń i inny udział kosztów pracy w przychodach.
Jednocześnie jeśli rośnie wycena procedury, czyli tego, ile NFZ płaci za konkretne świadczenie, to rośnie też wynagrodzenie lekarza na kontrakcie, który bierze procent od tej procedury.
W ubiegłym roku w przedstawionych rekomendacjach dotyczących podwyżki wynagrodzeń personelu medycznego nie zalecaliśmy podnoszenia stawek dla kontraktowców. Mimo to – według naszych analiz – płace lekarzy na kontraktach urosły o 8,5 proc., a 74 proc. lekarzy specjalistów rozlicza się ze szpitalami w taki sposób.
