A new study suggests that the link between excess body fat and cancer risk has been significantly underestimated. Researchers analyzing data from over 458,543 individuals found that more than one in ten cancer cases could be associated with excessive body fat, double the estimate previously used. The findings, published in the journal Cancer Communications, challenge long-standing assumptions about how obesity influences cancer development. The research was conducted by a team led by epidemiologist Hermann Brenner at the German Cancer Research Center (DKFZ). They analyzed data from the UK Biobank, one of the world’s largest health databases. After excluding participants with prior cancer diagnoses or incomplete information, the researchers focused on 458,543 adults, averaging around 57 years old, with nearly half being women. Over almost twelve years, 50,136 of these individuals developed cancer. The study measured overweight using three different methods: body mass index (BMI), waist circumference, and the waist-to-hip ratio. These metrics were evaluated in two ways: first, using standard weight categories, and second, by dividing participants into four equal groups based on their measurements, quartiles. This approach allowed for a more nuanced understanding of how varying levels of body fat might influence cancer risk. One key limitation of traditional BMI calculations is that they fail to distinguish between muscle and fat, nor do they indicate where fat accumulates in the body. Visceral fat, which surrounds internal organs, is metabolically active and linked to chronic inflammation, making it a stronger risk factor than subcutaneous fat located elsewhere. The study highlights that tail measurements and waist-to-hip ratios provide more accurate indicators of this type of fat distribution. To account for potential biases, the researchers adjusted for early cancer diagnoses. Some tumors can weaken the body years before detection, leading to lower recorded BMI values among those who later develop cancer. By removing cases diagnosed within the first few years of the study, the team aimed to eliminate this confounding effect. Combining all three refinements, the use of waist-based measures, quartile classification, and exclusion of early cases, led to a substantial shift in results. The proportion of cancer cases potentially attributable to excess body fat rose to 11.5 percent. In contrast, earlier estimates based solely on BMI yielded 5.5 percent, while older studies for Britain had estimated around 6.3 percent. These findings underscore the limitations of relying exclusively on BMI as a measure of health risk. While BMI remains a widely used tool due to its simplicity, the study reveals that it fails to capture the complex relationship between fat distribution and disease outcomes. The implications suggest that public health strategies targeting obesity should consider more detailed assessments of body composition rather than just overall weight.
