A coronial inquest into the death of a newborn baby at a Queensland hospital has concluded that her death resulted from an undiagnosed congenital heart condition rather than inadequate postnatal care. Thea Flaskett, who was born via vacuum-assisted delivery at Redcliffe Hospital in September 2023, died four hours later due to Transposition of the Great Arteries (TGA), a rare cardiac anomaly. The findings, released by Deputy State Coroner Stephanie Gallagher, exonerated the medical staff involved in her care, stating that their efforts were appropriate under the circumstances. Thea’s parents, James and Meg Flaskett, had raised concerns about their daughter’s treatment following her birth. These concerns led to an inquest held in Brisbane last year. The coroner’s final report, issued recently, confirmed that while the condition was not detected before birth, the care provided after delivery met acceptable standards. The inquest revealed that Thea’s condition, TGA, was not identified during prenatal scans. Experts testified that the failure to detect such conditions during antenatal screenings is not unusual. The coroner noted that the scans performed on Thea’s mother followed established protocols and did not require further investigation. The lack of detection was attributed to the inherent limitations of current antenatal screening techniques. Following Thea’s deterioration shortly after birth, hospital staff initiated immediate resuscitation efforts. A neonatal rescue team from the Royal Brisbane and Women’s Hospital (RBWH) was also dispatched to provide specialized care. According to the coroner’s findings, all personnel involved acted appropriately and did everything necessary to attempt to save Thea’s life. Despite these interventions, Thea’s survival was ultimately determined by the severity of her underlying cardiac condition. The coroner emphasized that even if the condition had been diagnosed prenatally, there was no certainty that her death could have been prevented. This conclusion underscored the unpredictable nature of certain congenital anomalies and the challenges faced by medical professionals in such cases. The coroner’s report included two key recommendations aimed at improving neonatal care practices. One recommendation urged Metro North Hospital and Health Services (MNHHS) and the NeoRESQ team at RBWH to investigate the resources available in New South Wales public hospitals. Specifically, they were advised to explore the feasibility of live-streaming consultations to consultants in pediatric resuscitation areas. This measure aims to enhance visibility and communication during critical moments in neonatal emergencies. Another recommendation focused on reviewing the intake forms used for neonatal retrieval services in Queensland. The coroner suggested examining the requirements outlined in similar forms used in New South Wales to determine if modifications could improve the efficiency and effectiveness of such services in Queensland. The findings highlight the complexities involved in diagnosing and managing rare congenital conditions. While the medical teams involved in Thea’s care were commended for their efforts, the coroner acknowledged the broader challenges within the healthcare system regarding early detection of such conditions. The recommendations aim to address potential gaps in current procedures and ensure that future cases benefit from improved practices and enhanced collaboration among medical teams.
